Regarding the first two questions; my vote is the usual thing but occasionally I will have trouble getting to sleep and it will take an hour or two before I finally do get to sleep. In which case the reasons are likely to be a mixture of fantasy/daydream and mentally going over today/tomorrow, with or without the physical discomfort thing if appropriate. It's not a common occurrence though, usually I sleep very quickly and heavily. (ie; I can sleep right through a car crash right outside my house)
Until recently it's been "More than an hour". Since the sleepcatcher thing, it's been closer to the 30-45 range, which is quite a switch. Because really, the old more than an hour was usually more than two, and sometimes more than three. About the getting up early thing, I usually change my sleeping habits by staying up 2 days and a night, and then collapsing at a place where a full nights sleep will let me wake up where I need to be up.
One more thing I forgot - the response to Q3 - "Not a chance - I get in a bedtime habit, and my body sticks to it" my habit currently is to go to bed about 1am, and up again at 8am. I find I'm doing better (feeling more awake during the day, and less depressed) on this 7-hour sleep pattern than I did on my previous 9. So although I get complains from my husband for going to bed late and disturbing him when he's already been asleep 2 hours, I'm in no hurry to change this pattern at the moment.
A good nights sleep, something that is very rare for me. Also, my dreaming is usually so vivid. I ususally have most of my sences in dreaming, that I wake still tired from a full nights dreaming.

Sounds like a more drastic change than I mean... I'm talking about 1-2, at most 3 hours earlier than usual, not converting from nocturnal to diurnal.
