Regarding the first two questions; my vote is the usual thing but occasionally I will have trouble getting to sleep and it will take an hour or two before I finally do get to sleep. In which case the reasons are likely to be a mixture of fantasy/daydream and mentally going over today/tomorrow, with or without the physical discomfort thing if appropriate. It's not a common occurrence though, usually I sleep very quickly and heavily. (ie; I can sleep right through a car crash right outside my house)
One more thing I forgot - the response to Q3 - "Not a chance - I get in a bedtime habit, and my body sticks to it" my habit currently is to go to bed about 1am, and up again at 8am. I find I'm doing better (feeling more awake during the day, and less depressed) on this 7-hour sleep pattern than I did on my previous 9. So although I get complains from my husband for going to bed late and disturbing him when he's already been asleep 2 hours, I'm in no hurry to change this pattern at the moment.
